Utility Forms

Setting up your utility account is easy and straightforward. You can either do it by downloading the Utility Account Application (link to forms page) and submitting via email or by visiting the Town of Fairview Administration office. We recommend filling out the form at least five days before your occupancy/possession date to ensure a seamless transition. Here's what you need to do:

  1. Provide your personal details and mailing information.
  2. Submit any required documents such as proof of purchase.
  3. Select a payment method that suits your needs; Pre-Authorized payment plans are available.

If you are a current resident moving within Fairview and need to move your town utilities, the process is the same as cancelling your current service. You will then need to complete a new account application. Accounts are not transferable between residences, contact our Utility administrator if you have any concerns with this process and allow yourself additional time to ensure a smooth process without any interruption.

If you are moving out of Fairview and need to cancel your utilities, fill out the Utility Account Cancellation form. We will process your request and ensure that your services are discontinued from the specified date. We recommend filling out the form at least five days before your move-out date.
